OVERVIEW

The purpose of this document is to define, in outline, the Health, Safety, Security and Environmental standards that are to be complied with by all the employees, contract laborers and third party personnel working with Flotek Engg Services.

Flotek HSSE policy simply stated is ‘No task or job is more important than Safety’. This principle is incorporated into all the projects executed by Flotek. The firm has adopted a zero tolerance policy toward any deviations from its core safety policy. The ultimate objective is to achieve zero injuries, accidents and environmental damages.

Any employee, who works for the firm in any project location and for whatever period of time, ought to align himself with the firm’s HSSE policy. In addition, any other mandate as required by the client’s HSSE policy needs to be followed by the employees while working in the client’s premises. It should also be noted that the client HSSE policy not necessarily supersedes that of the firm. Flotek HSSE policy should be followed irrespective of whether the client demands the same.

Health Safety Security Environment

Goals and Objectives

The following goals have been established at Flotek:

  • Provide workers with a safe work environment
  • Conduct routine/regular workplace inspections
  • Provide Personal Protective Equipment
  • Develop and implement safe work procedures and rules
  • Provide on-going safety training
  • Enforce safety rules and appropriate discipline
  • Provide on-going environment conservation practices
  • Cause no harm to the environment

Accidents - Treatment and Reporting

Any incident, accident, damage, near miss or any case of occupational hazard as a consequence of job execution – whether for Flotek or Client – must be immediately reported without any delay to the site engineer / supervisor, project-in- charge and representative of the client. This ensures all the incidents are investigated thoroughly and can be prevented in future.

The site emergency response team of client (if available) should be called immediately to provide the basic First Aid to the injured person. Else, available first aid at the site to be administered and the injured person shall be taken to the nearest medical facility having informed both the client and the project-in- charge.

In addition to accidents, all the Near Misses are to be reported periodically by the site engineer/ supervisor/safety officer employed by Flotek. This is a proactive measure which is used for identification of unsafe practices and conditions in the workplace. Even the near misses that seem to be trivial need to be reported so that they can be analyzed and used for prevention of future mishaps.

Personal Protective Equipment

General Requirements

During the execution of work in any premises on behalf of Flotek, all the employees / contract labor / third party personnel are obliged to wear the following protection as a minimum:

  • Safety helmet

  • High, antistatic Safety shoes

  • Fire retardant clothing (for applicable workers)

  • Gloves

  • Safety glasses with side protection

  • Safety Belt / Fall Arrester for all types of height works

Welding and Grinding Work

All welders and grinders are required to use a face shield in combination with the appropriate safety glasses. They are also required to don approved PPE in the form of helmet, leather coat, apron, steel-toed work boot, gloves, no cuff pants and other equipment as deemed necessary by the Safety Officer / Site Engineer. Mechanical ventilation is required at all welding points in the work site.

Necessary fire screen also needs to be provided and welding booths maintained if in case the welding and grinding works are carried out near the operational petroleum tanks and pipelines.

Painting Work

All employees and visitors are required to wear approved eye protection. Spray painters are required to wear company provided work clothing (disposable shirt/pants). Approved respirators are required during those times when spray painting, mixing, or clean-up is taking place. Steel-toed boots/shoes required of paint sprayers. All spray painting is to be conducted within the permissible premises. Mechanical ventilation system is to be in operation when spray painting takes place.

New Employee Induction

All the new employees will be required to attend the basic appropriate level of safety induction. The purpose of the same is to ensure all the new personnel understand and get accustomed to the HSSE policy of the firm.

Safety Supervision and periodic Internal Safety Audits / Inspections and Toolboxes

The responsibility of supervision on its own lies mainly with the Safety Officer / Site Engineer. The concerned person must be well equipped with all the safety norms. Prior and during the execution of work, toolbox talks need to be organized on a regular basis to empower the workers to work with necessary safety awareness and knowledge.

Waste Handling and Protection of Soil / Groundwater

Any person working for or on behalf of Flotek is responsible for managing, cleaning up and properly disposing off the waste material, resulting from work done or otherwise. For this purpose, small containers may be used and the waste to be disposed in the designated area. The work site should be free from all sorts of litter and every employee is responsible for ensuring the same.

The employees also should take necessary precautions for preventing the pollution of soil/ground water in the work site. This applies to client locations as well the firm’s own factory yard.

Dismissal

Flotek reserves the right to dismiss immediately any employee/sub-contractor if this person does not observe or refuse to comply with the firm’s HSSE policy

General Rules

  • All tools/equipment must be maintained in good condition. They should be accompanied by test certificates issued by valid bodies. Only appropriate tools shall be used for specific jobs
  • All guards must be kept in place
  • No spliced electrical cords/wiring allowed
  • Only authorized personnel can operate the equipment
  • No horseplay, alcohol, or drugs allowed on premises
  • Fire extinguisher must be always kept near the welding/hot work area
  • PPE must be worn as prescribed by management
  • No one is allowed to make changes to the scaffolding other than workers who are competent
  • Mobile phones should not be brought to work site
